Chronic Disease Self-Management Program
This 6-week IN PERSON program, led by certified peer counselor is proven to reduce pain, provide alternative medication options, and improve overall quality of life! The program is ideal for those recently diagnosed with arthritis, migraines, fibromyalgia, or back pain and related symptoms.
Learn how to manage commonly experienced symptoms including pain, fatigue, depression, stress, and anxiety; manage medications; set weekly actionable goals, improve eating habits; and much more.
This class will meet at the Southeast Neighborhood Senior Center for six consecutive Thursdays beginning September 7 and ending October 12, 2023. *THIS WORKSHOP IS A CLOSED EVENT FOR SOUTHEAST SENIOR CENTER MEMBERS ONLY*
With the expert guidance of two trained and certified Lay Leaders, you will learn how to manage common symptoms we experience while living with health conditions. Symptoms like pain, fatigue, stress, anxiety, shortness of breath, difficult emotions, depression, managing multiple medications, working with health care providers and within health care systems, and much more!
YOU will be empowered to Age Well no matter what condition(s) you are living with!
